Include the state with the city
A name such as Springfield does not uniquely identify a birthplace. Check city and state, keeping the county if it helps you verify the record. Your current address and phone time zone may be different from your birthplace.
Find the birth jurisdiction
The CDC vital-records directory is organised by state or territory, with New York City separate from the rest of New York State. Start where the birth happened. Check what information the available record contains; do not assume every copy includes a birth time.
A practical form check
Separate day, month and year before entering them; confirm the order of an ambiguous date such as 04/05. A time of 7:15 PM is 19:15. Use your birthplace even while travelling, and do not convert the time to UTC yourself.
What the reading can show
Mark the time as unknown if you do not know it; do not replace it with noon. The current reading shows positions and symbols, without calculating an ascendant or houses. Read the sky manual and prepare your map online. This guide does not advertise a local office or practitioner.
